Provider Pilot Agreement.
This agreement applies to provider applications and private-pilot participation. Submitting an application does not verify credentials, activate a profile, guarantee admission, or create a public listing.
Pilot relationship
Providers operate their own practices and control which requests they accept, their availability, service methods, prices, locations, and professional decisions. Nothing here is intended to create employment, partnership, agency, or a guarantee of referrals. The final legal classification, entity names, insurance terms, and jurisdiction-specific obligations require counsel review.
Provider commitments
You confirm that you are at least 18, have authority to submit the application, and will:
- Provide accurate, current business, contact, service, price, schedule, and credential information.
- Maintain every license, registration, permit, and insurance required for each jurisdiction and service offered.
- Work within professional scope, sanitation, consent, privacy, safety, and recordkeeping requirements.
- Respond promptly to requests and never represent a time as booked until both provider and customer confirm.
- Disclose your cancellation, no-show, travel, accessibility, and payment policies before customer confirmation.
- Use customer information only to evaluate, coordinate, and provide the requested service, not for unrelated marketing without separate permission.
- Avoid sexual, harassing, discriminatory, deceptive, unsafe, or illegal conduct and misleading health claims.
- Notify Match & Muscle promptly of credential restrictions, complaints that may affect safety, calendar disconnection, or material profile changes.
Profiles, calendars, and review
You grant Match & Muscle permission to display profile content and photos you submit and represent that you have the rights needed to do so. If you connect a private calendar feed, you authorize use of the feed to identify busy times and prevent conflicts; customers do not receive private event details. You may disconnect the feed.
Match & Muscle may independently check credentials and label the source and date. A provider website claim alone will not be displayed as official verification. Match & Muscle may pause or remove a profile or request while investigating accuracy, safety, legal, or pilot-operational concerns.
Pilot fees and future changes
The founding private pilot has a $0 Match & Muscle enrollment fee, $0 provider subscription, and $0 Match & Muscle commission. Provider-set service charges remain between provider and customer. No future platform fee will apply retroactively. Any future pricing requires advance written notice and the provider’s affirmative acceptance before it applies.
No guaranteed business
Match & Muscle does not guarantee leads, bookings, revenue, ranking, availability, customer conduct, or uninterrupted service. The final operator identity, term and termination mechanics, indemnity, liability allocation, governing law, dispute process, tax language, insurance minimums, and notices provision must be completed with counsel before public launch.
Privacy and safety
The Privacy Policy and Cancellation & Safety Policy are incorporated into this pilot agreement. Material future changes may require a new acceptance.
Credential and insurance evidence
Before receiving real customer bookings, provide official-source credential details for each service jurisdiction, expiration dates, and current insurance evidence. Keep policy limitations and exclusions accurate. Match & Muscle records checks and may request renewed evidence; this is not a guarantee of adequate insurance coverage. Minimum coverage levels, platform coverage, and any additional-insured requirements remain to be determined before public paid launch.
Price, consent, and refunds
Publish clear cancellation, no-show, and refund terms with a version identifier, and disclose any mobile travel fee. The customer must accept the complete service price and policy before confirmation. Do not add charges or change those terms without the customer’s new agreement. Obtain any professional treatment consent required for the service directly; platform booking consent does not replace treatment consent. In-platform payment collection and refund execution remain disabled during this pilot.
Safety holds and termination
A safety report may place your profile on hold for new bookings while the owner reviews the matter. A hold is a precaution, not a finding of misconduct. Cooperate with factual investigation and maintain relevant evidence. Existing appointments require individual review and communication. Provider and platform may end pilot participation, subject to handling existing commitments and legally required records. Responsibility for pending payments, final notice mechanics, indemnity, and liability terms require legal review.
